HOW HABITAT HEALTH WORKS
How It Works
Habitat Health’s PACE program is full-service, providing primary and specialty care, prescriptions, center activities, transportation, home care, and more, all under one offering.
Once you are enrolled, Habitat Health will be your sole service provider. In our PACE program, you can expect:
A full and individualized care plan assessment with a dedicated 11-person interdisciplinary team (IDT). Your IDT will include qualified health care professionals, such as a Primary Care Provider, Registered Nurse, Occupational Therapist, Registered Dietitian, and Home Care Coordinator;
A detailed explanation of Habitat Health’s benefits and services;
An introduction to many other Habitat Health services based on your care plan needs, which may include help with activities of daily living (ADLs) such as laundry or meal preparation, scheduling of specialist appointments, or support with filling prescriptions.
For qualified participants, our program takes care of all out-of-pocket costs for covered services.

The Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that offers an alternative to nursing home care for those who want to remain in their homes and communities, but could use more support to do so.

Who Is Eligible & Expected Cost
People may be eligible for PACE if they:
Are 55 years of age or older;
Located in our service area;
Meet the need for nursing-home-level care;
But wish to remain in your home or community and can safely do so with the assistance of Habitat Health.
Most participants receive all Habitat Health services at no cost to them. There are no lifetime limits and no deductibles, or copayments (copays) for any drug, service, or care approved by your Habitat Health care team.
What you pay for Habitat Health’s PACE program may depend on your financial situation:
If you are eligible for Medicaid, or dually eligible for both Medicare and Medicaid with no share of cost, you won't pay a monthly premium for PACE;
If you don't qualify for Medicaid, but you have Medicare, you'll be responsible for a monthly premium for PACE;
If you don't have Medicare or Medicaid, you can pay for PACE privately.

How to Enroll
A Habitat Health representative will be able to walk you through every step of the enrollment process. Beyond confirming your initial eligibility for our program, we will also explain the Habitat Health program, offer a tour of our center, and conduct a health assessment.
Habitat Health will help submit your PACE application to the state for final approval. You will be notified of their decision, and if they determine you do not qualify, you have the option to appeal. If your application has been approved and we are accepting new participants, you can enroll on the first day of the next month.
