Enrollment Process
Habitat Health’s PACE program is full-service, providing primary and specialty care, prescriptions, center activities, transportation, home care, and more, with a Habitat Health representative guiding you through every step of enrollment.
Check EligibilityThe Habitat Health team will confirm your initial eligibility, explain the program services, offer a tour of our center, and conduct a health assessment
Habitat Health will help submit your PACE application to the state for final approval
If your application has been approved and we are accepting new participants, you can enroll on the first day of the next month
Most participants receive all Habitat Health services at no cost to them
What you pay for Habitat Health’s PACE program may depend on your financial situation.
Check EligibilityThere are no lifetime limits and no deductibles, or copayments (copays) for any drug, service, or care approved by your Habitat Health care team.
