Why PACE?

The Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that offers an alternative to nursing home care for those who want to remain in their homes and communities, but could use more support to do so.

We focus on keeping people at home and in their communities by providing services at our centers or through home visits.

We provide individualized care plans, which may include help with activities of daily living (ADLs) such as laundry or meal preparation, scheduling of specialist appointments, or support with filling prescriptions.

We cover all expenses for patients, reducing out-of-pocket costs.

Your interdisciplinary care team (IDT) will include qualified health care professionals, such as a Primary Care Provider, Registered Nurse, Occupational Therapist, Registered Dietitian, and Home Care Coordinator

Habitat Health’s PACE program is full-service, providing primary and specialty care, prescriptions, center activities, transportation, home care, and more, with a Habitat Health representative guiding you through every step of enrollment.

The Habitat Health team will confirm your initial eligibility, explain the program services, offer a tour of our center, and conduct a health assessment

2PACE Application Submission

Habitat Health will help submit your PACE application to the state for final approval

3Enrollment

If your application has been approved and we are accepting new participants, you can enroll on the first day of the next month
