When you join Habitat Health, we will be your new insurance provider. All of your medical expenses will be handled by Habitat Health. There will be no out-of-pocket costs to you if the care you get is covered and provided by Habitat Health. You may be liable for the costs of unauthorized or out-of-PACE network agreement services.
