A Habitat Health Enrollment Nurse will assess if you meet nursing home level of care

Able to live safely in community

Wish to remain in your home or community and can safely do so with the assistance of Habitat Health.

If for some reason you do not qualify for PACE programming, our enrollment team will provide you with a list of alternative services in your area.
