We founded Habitat Health because the current approach to aging fails older adults, caregivers, providers, and health systems.
We've seen family members and loved ones worry about aging at home, while many caregivers feel burnt out balancing their own needs with those of children and parents. Health systems are also struggling to provide comprehensive, cost-effective care for the growing aging population.
Habitat Health offers high-quality health care that is deeply integrated into local communities and extends, as needed, into the home, enabling older adults to age comfortably where they want to be.
