FAQs

  • Program of All-Inclusive Care for the Elderly (PACE) is a joint Medicare and Medicaid program that provides all medical care and social services for older adults who qualify. This program’s goal is to help them continue living at home and in their community.

  • Habitat Health is a PACE health plan and provider. Our services include medical care, social services, day center activities, transportation, physical therapy, prescription management, and nutrition counseling. Habitat Health also helps arrange your labs and diagnostics, home care, dental services, and specialty care.

  • Habitat Health focuses on whole-person care for older adults, delivered at home and in communities so they can stay independent. We also work with trusted health care partners like Kaiser Permanente to bring together the best resources and local connections for each person’s needs.

  • There are no financial eligibility requirements to become a PACE participant. You are eligible for Habitat Health if:

    • You are 55 years of age or older;

    • Located in our service area (check if you qualify here);

    • Meet the need for nursing-home-level care; 

    • But wish to remain in your home or community and can safely do so with the assistance of Habitat Health.

    . Our enrollment team will contact you, share more information, and set up an in-person visit to help you through the process.

  • We enroll participants every month! There is no “enrollment period.” Once your application is approved, you will be enrolled on the 1st day of the next month.

  • It usually takes a couple of weeks to join Habitat Health, and we will need to meet with you in-person. Our enrollment team will help you through the process to make it easy and quick. 

  • When you join Habitat Health, we will be your new insurance provider. All of your medical expenses will be handled by Habitat Health. There will be no out-of-pocket costs to you if the care you get is covered and provided by Habitat Health. You may be liable for the costs of unauthorized or out-of-PACE network agreement services.

  • If you are eligible for Medicaid, or dually eligible for both Medicare and Medicaid with no share of cost, all premiums are covered and there is no cost for services approved by your care team. 

    If you only have Medicare and are not eligible for Medicaid, you will be responsible for a Medicaid monthly premium and your Medicare Part D premium. You can also pay for Habitat Health PACE services privately if you are not currently enrolled in Medicare or Medicaid services.

    Our enrollment team can help you figure out any out-of-pocket costs before you go through the enrollment process.

  • Each state has its own requirements for deciding if someone needs nursing facility level of care. This depends mostly on what support you need with activities of daily living (ADLs), like walking, bathing, and eating. Habitat Health’s team will help gather information on your needs as part of the enrollment process.

  • Once you join Habitat Health, you must get all of your health care from Habitat Health or from our approved providers. Your providers at Habitat Health include primary care, specialty care, pharmacists, physical therapists, and more, all working together to give you comprehensive, seamless care. 

    Habitat Health works with doctors and facilities you might already be familiar with, like Kaiser Permanente. If your doctor is already working with Habitat Health, and the care they provide is coordinated through PACE, then you may be able to continue seeing that same doctor. If you are already a patient of Kaiser Permanente, then all of your records and medical information will easily transfer to us.

  • Habitat Health will provide free transportation to and from our centers, as well as to any other appointments or services that are in your care plan.

  • At our centers, you’ll find daily activities and social events, and meals or snacks. The center is where you will get your medical care, with our interdisciplinary care teams creating a care plan just for you.   • Habitat Health centers are not residential or overnight facilities. They are community hubs for Habitat Health participants and their families.

  • Yes, if you need support at home, we will provide in-home care as part of your personal care plan. Your care team will decide if you need these services. 

  • Once you join Habitat Health, we will provide any needed in-home care as part of your enrollment benefits and individual care plan. Please contact us at 1-844-664-2248, TTY: 711 or email info@habitathealth.com for any additional information.

  • Habitat Health PACE centers are open 8am to 5pm, Monday through Friday, and are closed on national holidays. For urgent needs when we are closed, you can call our 24/7 help line at 1-844-664-2248, TTY: 71. Your personal care plan will also consider your 24-hour home needs. 

  • Habitat Health combines medical, social, and community care all in one place, making it easier for caregivers to manage their family members’ health needs.

  • Participants are able to disenroll at any time, for any reason. Please let us know if you would like to disenroll and we will help you do so. 

    Your disenrollment will start on the first day of the month after you request it. We will keep providing services until then, but you will still need to use our providers and pay any premiums you are responsible for.
