Our Story

We founded Habitat Health because the status quo of aging isn’t currently working for older adults, their caregivers, providers, or health plans and systems.


Surveys show that older adults are worried about aging and most want to age at home. At the same time, caregivers are feeling burnt out and providers, health plans, and health systems are struggling to care comprehensively and cost-effectively for growing numbers of aging Americans. Habitat Health offers high-quality health care that is deeply integrated into local communities and extends, as needed, into the home, enabling older adults to age comfortably where they want to be.

At Habitat Health, our mission is to empower older adults to maintain independence and well-being. We achieve this through innovative partnerships with leading health care organizations, bringing together their clinical experience with our personalized, coordinated care program to meet your needs.

What Makes Us Different

  • Habitat Health is focused on providing community for participants, in addition to and beyond the medical and social care offered by other PACE programs. This means regular social opportunities, active community engagement, and an emphasis on remaining within their homes, as well as focusing on provider happiness.
